Addiction treatment is not one thing. It is a series of levels of care, and the single most common failure in this industry is a person being placed at whatever level a facility happens to sell rather than the level a clinical assessment indicates. A proper assessment looks at withdrawal risk, medical and psychiatric stability, the home environment, and what has already been tried.

One point that is easy to miss: detox is not treatment. Completing withdrawal and going home has poor outcomes, and with opioids it measurably raises overdose risk, because tolerance falls while nothing else about the person’s life has changed. Detox is the first week of a plan, not the plan.
